Pet Traveling Hotel – Professional Pet Transportation Services

Pet Information (Required)

• Puppies must be at least 8 weeks old at the time of pickup
• Clients must provide accurate and complete pet details prior to pickup
• This information is required for guest safety, proper crate sizing, route planning, and regulatory compliance
• Incorrect, incomplete, or missing pet information or required documents at pickup may result in delays, additional fees, or cancellation of the trip, with the deposit retained

Please provide the following guest information:
• Pet’s name
• Gender (male / female)
• Age (months / years)
• Weight (lbs or kg)
• Either a valid health certificate or updated vaccination records

Guests are welcome to bring personal items such as their own crate, favorite toys, or special snacks, if desired.

If applicable:
• Spay / neuter status
• Medical conditions or medications
• Behavioral notes (crate training, anxiety, aggression, compatibility with other pets)

Payment & Booking Information

Accepted payment methods:
• Zelle
• Cash
• Credit card (3.5% processing fee)

Trip Tracking & Delivery Timeline (Optional)

• Optional live GPS tracking link available for $50
• Real-time journey tracking from check-in (pickup) to check-out (delivery)
• Typical travel time is approximately 60 hours per 1,000 miles

Scheduling & Availability (Binding Time Windows)

• Estimated pickup and delivery time windows are provided at least 24 hours in advance and are binding
• The Client is responsible for being available within the confirmed time window, not the entire day
• Requests to delay pickup or delivery outside the confirmed time window may result in waiting fees, rescheduling, or cancellation at the driver’s discretion due to route scheduling constraints

Booking & Deposit

• A 50% deposit is required to reserve your pet’s space in our Pet Traveling Hotel, with a minimum of $200
• The remaining balance is due at delivery

Cancellation Policy

• More than 72 hours before pickup: Full deposit refund
• 24–72 hours before pickup: 50% of the deposit is retained (minimum $200)
• Less than 24 hours before pickup: Deposit is non-refundable

Final Notes – Important Terms

• Rates include insurance, fuel, tolls, vehicle maintenance, and mileage, with no hidden or surprise fees
• In the event of a medical emergency, the client authorizes Paws & Pathways Transport LLC to seek veterinary care if necessary; all veterinary expenses remain the responsibility of the client

• Delivery times are estimates and may vary due to weather, traffic, road conditions, or unforeseen circumstances
• If the client or authorized recipient is unavailable at the agreed delivery time, a waiting fee of $50 per hour will apply
• Paws & Pathways Transport LLC is not responsible for conditions related to pre-existing health issues, age-related complications, or stress-related responses during transport
• By accepting these terms, the Client acknowledges that pickup and delivery hours are between 8:00 AM and 12:00 AM
• Any undisclosed aggression, biting history, or severe behavioral issues may result in immediate cancellation without refund
• Booking confirms acceptance of all terms and conditions
